Is there a difference between having a car registered under 2 names vs. under one name?

The difference in having the policy listed in 2 insured's names instead of one is that if there is a loss, the claims payment would be made out in both names. Also, any claims under the policy would fall under both insureds and would stay on their records.

The named insured under the policy must always match the name(s) listed on the vehicle ownership.
